In order to get the application forms necessary for CDA Assessment or Renewal go to the following site (click the blue CDA button) and order these.  The purchase of these is non reimbursable.

The following must be submitted to Training in the CDA Assessment packet.
 
bulletThe completed CDA Assessment form (note that the specific training areas - the blanks to be filled in with training hours must equal 120 hours)
bulletA transcript for a college of other documentation of the specific training required for the CDA
The following must be submitted to Training in the CDA Renewal packet.
 
bulletThe completed CDA Renewal form
bulletDocumented proof of at least 4.5 Continuing Education Units (CEUs) or a three-credit-hour course in early childhood education/child development, principles of adult learning, mental health counseling, etc. These hours must be in addition to the original 120 clock hours required when the Candidate obtained the CDA Credential. Bilingual CDAs must meet this requirement with coursework incorporating bilingual issues
bulletDocumented proof of a current Red Cross or other agency First Aid Certificate
bulletDocumented proof of recent (within past year) work experience with young children or families of young children (a minimum of 80 hours) - (This letter can be written by a supervisor or Human Resources.)
bulletA completed Letter of Recommendation Form regarding the CDA's competence with young children prepared by an Early Childhood Education Professional - (This letter must be written by a supervisor.)
bulletDocumented proof (within current year) of membership in a national or local early childhood professional organization. A list of national organizations is included in the renewal application packet and appears in another section of this website: see “CDA Resources—Early Childhood Organizations”

Because Training is required to keep CDA records on all of our Teachers, please do the following:

 print the materials needed

 fill them out

bullet

 put the completed materials in an envelope

bullet

 and submit this to Training. 

Be sure to make copies of all the materials you submit.  Unfortunately, within our own system and at the CDA Council packets have been lost.  This is very rare, but if yours is lost, regardless of who lost it, you will have to replace it.

Training will have a check cut to pay for your CDA application or CDA renewal, and Training will mail this to the CDA Council.

Do not fill out these forms online.  We do not have an account with the CDA Council and they cannot bill us for payment.  If you pay online with your credit card we are not obligated and may not reimburse you.
